The WHO Analgesic Ladder Explained for Patients

The World Health Organization analgesic ladder is a simple idea that has relieved pain for millions: match the strength of the medicine to the severity of the pain, give it by mouth where possible, by the clock rather than only when pain becomes unbearable, and tailor it to the individual.

Step one: mild pain

Paracetamol and anti-inflammatory medicines, with adjuvants added if nerve pain is present. Regular dosing works far better than waiting for pain to peak.

Step two: moderate pain

Weaker opioids such as tramadol or low-dose combinations are added to — not substituted for — step one medicines.

Step three: severe pain

Strong opioids such as morphine, in a regular long-acting form plus an immediate-release rescue dose for breakthrough pain. Doses are titrated upwards until comfort is achieved or side effects limit progress.

The fourth step your doctor may add

Modern practice adds interventional procedures — nerve blocks, epidural or intrathecal techniques, radiofrequency — for pain that medicines cannot control comfortably. Waiting until every drug has failed is no longer good practice.

Three rules that matter most at home

• By the clock: take regular doses at fixed times, not only when pain returns
• By the mouth: tablets and liquids are preferred; injections are not stronger
• Rescue doses: keep an immediate-release dose ready for breakthrough pain, and record how often you need it
