In the Indian medical landscape, paracetamol and non-steroidal anti-inflammatory drugs (NSAIDs) are frequently the first step in the WHO Analgesic Ladder. When used correctly, they can reduce the requirement for higher doses of opioids and improve overall quality of life. However, their familiarity often leads to misuse or underestimation of their potency and potential side effects.

The Role of Paracetamol in Pain Control

Paracetamol, also known as acetaminophen, is one of the safest and most effective baseline analgesics available when taken at appropriate doses. It works primarily within the central nervous system to elevate the pain threshold. Unlike other painkillers, it is gentle on the stomach lining, making it a preferred choice for patients who suffer from acidity or gastritis. In palliative care, we often prescribe it on a regular 'by the clock' schedule rather than only when pain becomes unbearable, as this maintains a steady level of the drug in the bloodstream to prevent pain spikes.

For most adults with normal liver function, the standard dose is 500mg to 1000mg every six hours. It is crucial to respect the maximum daily limit of 4000mg, as exceeding this can lead to serious liver toxicity. Patients must be vigilant about 'hidden' paracetamol in combination medicines for colds, coughs, or flu, which could inadvertently lead to an overdose. When combined with other analgesics, paracetamol exhibits a synergistic effect, meaning the drugs work better together than they would individually.

Understanding NSAIDs and Inflammation

Non-steroidal anti-inflammatory drugs, such as ibuprofen, diclofenac, and naproxen, function differently from paracetamol. They work by blocking enzymes that produce prostaglandins, the chemicals responsible for inflammation and pain at the site of injury or disease. This makes them particularly effective for bone pain, metastatic discomfort, and inflammatory conditions. In the context of cancer pain, NSAIDs are often indispensable for managing the localized inflammation caused by a growing tumour or the side effects of certain treatments like radiation.

While highly effective, NSAIDs must be used with greater caution than paracetamol. They can affect the kidneys, increase the risk of gastric ulcers, and occasionally impact blood pressure or heart health. We typically advise taking these medications after meals to protect the stomach lining. For patients at higher risk of gastrointestinal issues, we may co-prescribe a proton pump inhibitor to reduce acid production and safeguard the digestive tract.

This is particularly beneficial as it helps minimise opioid-related side effects such as severe constipation, drowsiness, and nausea, which can significantly impact a patient's daily functioning and alertness.

In many cases, the combination of these basic drugs acts as a safety net. If a patient misses a dose of their stronger medication, the baseline paracetamol or NSAID continues to provide a layer of protection against breakthrough pain. This multimodal approach is the gold standard in modern pain medicine, ensuring that we are not relying on a single mechanism to provide relief, but rather attacking the pain from various physiological angles simultaneously.

Safety Considerations and Contraindications

Patients must be aware of specific conditions where these drugs should be avoided or used under strict supervision. For instance, individuals with chronic kidney disease, active stomach ulcers, or severe heart failure should generally avoid NSAIDs. Similarly, those with significant liver dysfunction must be cautious with paracetamol. It is always important to disclose your full medical history, including any history of asthma or allergic reactions to aspirin, before starting a new regimen of anti-inflammatory drugs.

  • Monitor for signs of gastric distress, such as dark stools or persistent indigestion.
  • Watch for swelling in the ankles or decreased urine output, which may indicate kidney strain.
